Dogs are social animals who learn from each other, both good and bad. Even if only one dog needs training, the behavior of your other dogs may be affecting the “problem child.”
If you have more than one dog, please complete this form for EACH of your dogs.
If you only have one dog, you don’t need to fill out this form. Just complete the detailed BHH (Behavior Health Habits) Questionnaire on that dog.
For example, suppose you have 3 dogs — Arby, Bella, and Champ. Arby is your new dog that you want trained. You’ve had Bella and Champ for years and don’t need them trained. You would fill out the Multiple Dog Household form twice — first you’d fill it out on Bella and submit it. Then fill it out again about Chance and submit it. You wouldn’t need to fill it out on Arby because you’ll fill out the longer BHH (Behavior Health Habits Questionnaire) on Arby.
If you have more than one dog, please fill out a copy of the form below for each of the dogs in your household.
Make sure, after you hit “submit,” to check that the form was submitted. Sometimes there are blank fields or other “errors” that need to be corrected. Otherwise the form will not get to your trainer.
If you haven’t already, remember to fill out the detailed history (BHH form) on the dog(s) that need training.
